Registration with the Practice

If you live within our Practice Boundary we will be happy to welcome you as a new patient. If you require information about the practice prior to registration the Practice Manager, Sandy Pearson, will be pleased to help you. You can pre-register on-line using this website.  You will be required to come in to sign your registration forms to complete your registration with us.  You can also register quickly and easily by bringing your medical card or registration form to reception.  A printable registration form is available on the website

You will be requested to have a short appointment with one of our nursing team so we can take a medical history from you and perform a brief health check. When registering with the practice you may specify a preferred doctor and we will endeavour to comply with any reasonable preference.

Practice Area

The practice is centrally situated in the city centre and the practice area includes Lancaster and some surrounding areas south of the River Lune. The practice boundary is also shown on a map in the reception area.

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. You will need to come to reception to sign your registration forms to complete your registration with us, and to book an appointment for a brief health check with a member of the nursing team.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.
